Von Willebrand disease
Von Willebrand disease (vWD) is the MOST COMMON inherited bleeding disorder, caused by quantitative and/or qualitative problems in von Willebrand factor (vWF), often with secondary low factor VIII. Bleeding is typically MUCOSAL (epistaxis, menorrhagia, gum bleeding) and procedure-related. Management: (1) DDAVP (desmopressin) in responsive patients, (2) vWF concentrates when DDAVP unsuitable/ineffective, (3) tranexamic acid (TXA) as adjunct, (4) local hemostatic measures always. ASH/ISTH/NHF/WFH 2021 guideline is the primary reference. Key dental principle: the special protocol is HEMOSTASIS, not antibiotics.
