Pregnancy (3rd trimester) — supine hypotension + late pregnancy constraints
In late pregnancy (3rd trimester, ~28-40 weeks): (1) SUPINE HYPOTENSIVE SYNDROME — IVC compression → hypotension/syncope; left lateral tilt. (2) HYPERTENSIVE DISORDERS — gestational HTN, pre-eclampsia (severe: BP ≥160/110), eclampsia (seizure = emergency). (3) HIGHER ASPIRATION RISK — delayed gastric emptying, incompetent LES, full-stomach tendency → regurgitation risk especially with sedation; keep chair more upright. (4) REDUCED TOLERANCE — dyspnea, back discomfort, shorter appointments needed. (5) NSAIDs CONTRAINDICATED. ADA/ACOG: necessary dental care remains SAFE; LA acceptable.
