Antibiotic prophylaxis for IE prevention is NOT routine — reserved for a small HIGHEST-RISK CARDIAC GROUP when doing specific invasive dental procedures. AHA wallet card + ADA: prophylaxis recommended only for patients with conditions associated with highest risk of adverse outcome, only for procedures involving gingival/periapical manipulation or mucosal perforation. KEY: oral hygiene is the MAIN prevention strategy; prophylaxis is supplementary for the highest-risk subset only.
