Hypertension (uncontrolled / hypertensive urgency)
Hypertension = sustained office BP ≥130/80 mmHg (US: ACC/AHA 2017/2025; UK/Europe often use ≥140/90). Uncontrolled or severe hypertension raises the risk of stroke, MI, and hypertensive crisis during stressful dental procedures. Separate two distinct problems: (1) asymptomatic elevated/severe BP — a threshold question of whether to proceed with or defer elective care; and (2) a true hypertensive EMERGENCY — severe BP (≥180/120 mmHg) WITH acute target-organ signs — which needs prompt EMS, not chairside BP-lowering. The routine concern is whether BP is controlled enough to proceed safely.
