Why this matters

Almost every clinical problem you will meet in the mouth is a vascular or lymphatic problem wearing a different name. Local anaesthetic works longer because a drug constricts arteriolar muscle. A needle in the pterygomandibular space enters a vessel in 55 of 359 recorded injections. A pulp survives or dies according to what happens in vessels inside a rigid chamber. Facial swelling after surgery is a drainage failure. A neck lump that does not settle is a lymph node telling you something.

The big picture

One tube changes its wall along its length: elastin where it must recoil, muscle where it must regulate, almost nothing where it must exchange, valves where pressure is too low to push. What leaks out does not go back in — it leaves by a second, blind-ended system running through filters, and those filters are where immunity meets the mouth.

Media names the artery, adventitia the vein, and what leaks out leaves by lymph.
