Poor soft-tissue management (thin biotype, no keratinized tissue plan)
What it is
Failing to evaluate and manage the peri-implant soft tissue biotype (thin vs thick), width of keratinized mucosa, and tissue thickness at the implant site. This leads to recession exposing the implant collar/abutment, mucogingival problems, and compromised esthetics and hygiene.
Why it happens
• Focusing only on bone and ignoring soft tissue assessment • Not measuring keratinized tissue width pre-operatively • Placing implants in thin biotype without connective tissue graft • Poor flap design that sacrifices keratinized tissue • Not planning for soft tissue augmentation when needed • Assuming soft tissue will adapt after implant placement
