What it is
Designing an implant restoration with an emergence profile that does not allow adequate oral hygiene access. The transition from the implant platform to the crown contour is too abrupt, too bulky, or creates concavities that trap plaque and are impossible to clean with floss or interdental brushes.
Why it happens
• Implant placed too deep subcrestally without planning the transmucosal profile • Wide implant platform with narrow ridge creates excessive soft tissue draping • No provisional shaping of the soft tissue (skipping the provisional phase) • Lab communication failure: technician not informed of soft tissue contours • Using stock abutments instead of custom-contoured abutments • Restoring immediately without gradual tissue conditioning
